Function

Amylase is an enzyme produced primarily by the pancreas and the salivary glands. Its main job is to help the body break down complex carbohydrates (starches) into simple sugars. While small amounts of amylase are normally found in the blood, a large 'leak' of this enzyme into the bloodstream usually indicates damage to the cells that produce it.

Why it is Ordered

The amylase test is most frequently used to diagnose and monitor acute pancreatitis (inflammation of the pancreas). It is often ordered alongside a Lipase test when a patient experiences severe abdominal pain, fever, loss of appetite, or nausea. It can also help evaluate conditions affecting the salivary glands.

Associated Conditions

  • Acute Pancreatitis: Levels often rise 4-6 times higher than the upper limit.
  • Pancreatic Cancer or Duct Obstruction: Caused by gallstones blocking the pancreatic duct.
  • Mumps: Inflammation of the salivary glands increases salivary amylase.
  • Macroamylasemia: A benign condition where amylase binds to proteins in the blood, causing high levels without disease.

Why Context Matters

Serum amylase can be elevated by factors unrelated to the pancreas, such as kidney failure (reduced clearance) or the use of certain medications (opiates, thiazide diuretics, oral contraceptives). Additionally, 'Macroamylasemia' can lead to chronically high amylase levels that are completely harmless. A second opinion should focus on correlating amylase with lipase levels and imaging (CT/Ultrasound) to avoid misdiagnosis.

Lab ranges are statistical averages, not biological laws. "Normal" for a 20-year-old male isn't normal for a 60-year-old female.
