This combined test measures glucose levels at two distinct states: Fasting Blood Sugar (FBS), measured after at least 8 hours of no caloric intake, and Post-Prandial Blood Sugar (PPBS), measured exactly 2 hours after a meal. Together, they assess the body's ability to maintain glucose homeostasis and the effectiveness of insulin response to a glucose load.

  • Screening for Type 2 Diabetes and Pre-diabetes.
  • Monitoring glucose control in known diabetics.
  • Evaluating 'Reactive Hypoglycemia' (low sugar after eating).

Persistent elevations in FBS and PPBS are diagnostic of Diabetes Mellitus. Pre-diabetes is identified when levels are higher than normal but not yet in the diabetic range. Chronic hyperglycemia is associated with long-term damage to the heart, kidneys (nephropathy), nerves (neuropathy), and eyes (retinopathy). Conversely, consistently low FBS levels may indicate an insulinoma (insulin-secreting tumor) or hormonal deficiencies like Addison’s disease.

Why Context Matters

Glucose levels are highly sensitive to recent lifestyle factors. A second opinion/test is vital if the patient did not fast strictly for 8-12 hours or if the 'post-prandial' meal did not contain an adequate carbohydrate load (standardized at 75g in some settings). Acute stress, illness, or surgery can cause temporary 'stress hyperglycemia.' Additionally, medications like steroids (prednisone) can significantly elevate glucose levels, while intense exercise right before the test can lower them.

Lab ranges are statistical averages, not biological laws. "Normal" for a 20-year-old male isn't normal for a 60-year-old female.
