The MGMT (O6-methylguanine-DNA methyltransferase) test measures the methylation status of the promoter region of the MGMT gene. The MGMT gene produces a protein that repairs DNA damage. In the context of cancer treatment, specifically glioblastoma, this repair mechanism can actually be detrimental because it 'fixes' the DNA damage caused by chemotherapy drugs like Temozolomide.
Why it is Ordered
This is a predictive biomarker test. If the MGMT promoter is 'methylated,' the gene is essentially switched off. This means the tumor cells cannot repair the damage caused by chemotherapy, making the treatment much more effective. If the promoter is 'unmethylated,' the gene is active, and the tumor is more likely to be resistant to standard chemotherapy.

Why Context Matters
The interpretation of MGMT status can be complex because methylation is not always 'all or nothing.' There are 'gray zone' or 'weakly methylated' results that vary depending on the laboratory's threshold or the specific technique used (e.g., PCR vs. Sequencing). A second review by a neuropathologist is often beneficial for borderline cases.
Lab ranges are statistical averages, not biological laws. "Normal" for a 20-year-old male isn't normal for a 60-year-old female.
