Function

The Papanicolaou (Pap) smear is a screening tool used to detect precancerous and cancerous changes in the cells of the cervix. It identifies cytological abnormalities that might indicate Human Papillomavirus (HPV) infection or early-stage cervical cancer.

Why it is Ordered

It is the primary screening method for cervical cancer prevention. Regular screening allows for the detection of Abnormal Squamous Cells (ASC) or Intraepithelial Lesions (SIL) long before they develop into invasive cancer, at a stage where they are highly treatable.

Associated Conditions

  • Cervical Cancer: Early detection of squamous cell carcinoma or adenocarcinoma.
  • HPV Infection: Identifying cellular changes (koilocytosis) associated with high-risk HPV types.
  • Dysplasia: Grading the severity of cervical intraepithelial neoplasia (CIN).

Why Context Matters

Several factors can lead to 'unsatisfactory' results or false negatives/positives. These include recent sexual intercourse (within 48 hours), use of vaginal creams or douches, or being on a menstrual period, which can obscure cellular detail with blood and mucus. Inflammation or localized infections (like yeast infections) can also mimic dysplastic changes, necessitating a second opinion or a follow-up test after treatment of the infection.

Lab ranges are statistical averages, not biological laws. "Normal" for a 20-year-old male isn't normal for a 60-year-old female.
