The ROS1 Immunohistochemistry (IHC) marker is a specialized laboratory test performed on tumor tissue to detect the overexpression of the ROS1 protein. ROS1 is a receptor tyrosine kinase; in certain cancers, the ROS1 gene undergoes a rearrangement (fusion) with other genes, leading to constitutive activation of the protein. This activation drives uncontrolled cell growth and survival. The IHC test uses specific antibodies that bind to the ROS1 protein, producing a color change visible under a microscope.

This test is a critical component of 'Precision Medicine' for patients diagnosed with Non-Small Cell Lung Cancer (NSCLC), specifically those with adenocarcinoma histology. Identifying a ROS1 rearrangement is transformative for treatment, as these patients typically do not respond well to traditional chemotherapy but show dramatic, durable responses to targeted therapies known as Tyrosine Kinase Inhibitors (TKIs), such as Crizotinib or Entrectinib.

Why Context Matters

IHC for ROS1 is highly sensitive but can occasionally produce false positives (low specificity) due to background staining or the presence of the normal ROS1 protein in certain tissues. Because a positive result commits a patient to expensive and specific targeted therapy, many clinical guidelines recommend confirming a positive IHC result with a more definitive method like Fluorescence In Situ Hybridization (FISH) or Next-Generation Sequencing (NGS) to verify the actual gene fusion.

Lab ranges are statistical averages, not biological laws. "Normal" for a 20-year-old male isn't normal for a 60-year-old female.
