Function

The Synovial Fluid Culture and Sensitivity (C/S) test is a critical diagnostic procedure used to identify infectious organisms within a joint space. Synovial fluid acts as a lubricant for joints; however, when bacteria, fungi, or viruses infiltrate this space, it leads to septic arthritis—a medical emergency. This test involves inoculating the fluid into growth media to see if pathogens multiply and then testing those pathogens against various antibiotics.

Why it is Ordered

Physicians order this test when a patient presents with symptoms of an acutely inflamed joint, such as severe pain, swelling, warmth, and restricted range of motion. It is the gold standard for differentiating between infectious (septic) arthritis and non-infectious causes like gout, rheumatoid arthritis, or osteoarthritis. Timely diagnosis is vital to prevent permanent joint destruction or systemic sepsis.

Associated Conditions

  • Septic Arthritis: Most commonly caused by Staphylococcus aureus or Neisseria gonorrhoeae.
  • Chronic Granulomatous Infections: Such as fungal infections or tuberculosis in the joint.
  • Post-Surgical Infections: Infections following joint replacement (arthroplasty).
  • Crystal-Induced Synovitis: While C/S is for infection, the procedure often includes crystal analysis to rule out gout or pseudogout.

Why Context Matters

A second opinion or repeat aspiration may be necessary if the initial culture is negative despite high clinical suspicion (e.g., high white cell count in the fluid). Confounding factors include prior antibiotic use before the sample was taken, which can suppress bacterial growth, or fastidious organisms that require specialized growth media not used in standard protocols.

Lab ranges are statistical averages, not biological laws. "Normal" for a 20-year-old male isn't normal for a 60-year-old female.
