Function

Bilirubin is a yellowish pigment formed during the normal breakdown of red blood cells (hemolysis). Total Bilirubin measures the combination of 'unconjugated' (indirect) bilirubin, which travels in the blood to the liver, and 'conjugated' (direct) bilirubin, which has been processed by the liver for excretion into the bile.

Why it is Ordered

This test is a staple of Liver Function Tests (LFTs). It is used to diagnose and monitor liver diseases such as hepatitis or cirrhosis, evaluate potential bile duct obstructions (gallstones), and investigate the cause of jaundice (yellowing of the skin and eyes). It is also used to monitor newborns for neonatal jaundice.

Why Context Matters

Prolonged fasting or strenuous exercise can cause a transient rise in bilirubin, particularly in those with Gilbert's Syndrome. Conversely, exposure of the blood sample to strong light can degrade bilirubin, leading to falsely low results. High levels of Vitamin C or certain antibiotics can also interfere with the chemical assay.

Lab ranges are statistical averages, not biological laws. "Normal" for a 20-year-old male isn't normal for a 60-year-old female.
