Anti-double-stranded DNA (anti-dsDNA) antibodies are a type of antinuclear antibody (ANA) that specifically target the genetic material (DNA) inside the cell nucleus. They are highly specific markers for autoimmune processes, where the immune system loses the ability to distinguish self from non-self.
Why it is Ordered
This test is primarily used to support a diagnosis of Systemic Lupus Erythematosus (SLE). It is often ordered as a follow-up to a positive ANA test. Because anti-dsDNA levels often fluctuate with disease activity, it is also used to monitor 'flares' and the risk of kidney involvement (Lupus Nephritis).
Associated Conditions
- Systemic Lupus Erythematosus (SLE): Highly specific; ~70% of SLE patients test positive.
- Lupus Nephritis: High titers of anti-dsDNA are strongly associated with inflammation of the kidneys.
- Drug-Induced Lupus: Some medications can cause a lupus-like syndrome, though anti-dsDNA is less common here than anti-histone antibodies.
Clinical Correlation
While a positive result is strong evidence for SLE, a negative result does not rule it out. Diagnosis requires a combination of clinical symptoms (rash, joint pain, kidney issues) and laboratory findings.

Why Context Matters
Different laboratory methods (ELISA, Crithidia luciliae IFA, or Farr assay) have different sensitivities and specificities. A borderline result on an ELISA might need confirmation by the more specific Crithidia assay. Additionally, certain viral infections or drugs (e.g., Hydralazine, Procainamide) can cause transient elevations.
Lab ranges are statistical averages, not biological laws. "Normal" for a 20-year-old male isn't normal for a 60-year-old female.
