HBeAg (Hepatitis B e-Antigen) is a soluble protein that is a byproduct of Hepatitis B Virus (HBV) replication. Its presence in the blood serves as a marker of active viral replication and high levels of HBV DNA in the serum.

Why it is Ordered

This test is used to manage patients already diagnosed with Chronic Hepatitis B. It helps determine how infectious the patient is (contagiousness) and monitors the effectiveness of antiviral therapy. A 'seroconversion' from HBeAg-positive to HBeAg-negative (and HBeAb-positive) is often a primary goal of treatment, signaling a transition to a less active state of the virus.

Associated Conditions

  • Chronic Hepatitis B (Active): High viral load and high risk of liver damage.
  • Acute Hepatitis B: Present during the early stages of infection.
  • HBeAg-Negative Chronic Hepatitis: A variant where the virus has mutated and does not produce the e-antigen despite high replication; this is a more aggressive form of the disease.

Why Context Matters

HBeAg results must be interpreted alongside HBsAg, Anti-HBe, and HBV DNA levels. A negative HBeAg does not always mean the virus is inactive (due to precore mutants). Additionally, biotin (Vitamin B7) supplements can interfere with the immunoassay used to detect this antigen, leading to false results. If clinical symptoms of liver distress (jaundice, high ALT) persist despite a negative HBeAg, further molecular testing is required.

Lab ranges are statistical averages, not biological laws. "Normal" for a 20-year-old male isn't normal for a 60-year-old female.
