The Hepatitis E Virus (HEV) IgM test is a serological assay used to detect Immunoglobulin M antibodies against the Hepatitis E virus. IgM is the first antibody the body produces when it encounters a new infection. Its presence in the blood is a hallmark of an acute or very recent infection, as these antibodies typically disappear within 3 to 6 months after the initial exposure.
Why it is Ordered
This test is ordered for patients exhibiting signs of acute viral hepatitis, especially if they have recently traveled to endemic areas (Asia, Africa, Central America) or have consumed undercooked pork or venison. Symptoms often include jaundice (yellowing of eyes and skin), dark urine, extreme fatigue, and abdominal pain. It is particularly vital for pregnant women, as HEV infection in the third trimester can lead to fulminant hepatic failure with high mortality rates.

- Acute Hepatitis E: The primary condition identified by a positive IgM.
- Chronic Hepatitis E: Rare, but can occur in immunosuppressed individuals (though IgM may be negative here).
- Zoonotic Transmission: Infection via contaminated food sources.

Why Context Matters
False positives can occur due to cross-reactivity with other viruses like Hepatitis A, Epstein-Barr Virus (EBV), or Cytomegalovirus (CMV). Furthermore, in immunocompromised patients, the body may fail to produce a robust IgM response, leading to a false negative despite an active infection. A second opinion involving HEV RNA PCR testing is often recommended for confirmation.
Lab ranges are statistical averages, not biological laws. "Normal" for a 20-year-old male isn't normal for a 60-year-old female.
