Function

The Autoimmune Hepatitis (AIH) Panel is a comprehensive group of tests designed to detect various autoantibodies that characterize immune-mediated liver disease. Unlike a single test, the panel looks for multiple markers including Antinuclear Antibody (ANA), Smooth Muscle Antibody (ASMA), Liver-Kidney Microsome (LKM-1) antibody, and sometimes Soluble Liver Antigen (SLA).

Why it is Ordered

Diagnosis of AIH is complex because its symptoms—fatigue, nausea, and joint pain—overlap with many other liver diseases, including viral hepatitis and fatty liver. This panel is ordered to differentiate AIH from these other conditions. It helps clinicians classify the disease into Type 1 (ANA/ASMA positive) or Type 2 (LKM-1 positive), which determines the treatment approach and prognosis.

Associated Conditions

  • Autoimmune Hepatitis (AIH): The primary condition. Type 1 is more common in adults; Type 2 is common in children.
  • Primary Biliary Cholangitis (PBC): Sometimes markers overlap, though PBC usually features Anti-Mitochondrial Antibodies (AMA).
  • Systemic Lupus Erythematosus (SLE): ANA is positive in SLE and can sometimes be associated with 'Lupoid Hepatitis.'
  • Infectious Hepatitis: Hepatitis C can occasionally cause positive results on these panels without AIH being present.

Why Context Matters

Autoimmune panels are notorious for 'low-titer' false positives. For example, a low ANA or ASMA titer (e.g., 1:40) can be found in up to 15% of the healthy population or during minor viral illnesses. A second opinion ensures that the patient is not misdiagnosed with a chronic autoimmune condition based solely on a low-positive titer without supporting evidence from liver biopsies or significantly elevated ALT/AST levels.

Lab ranges are statistical averages, not biological laws. "Normal" for a 20-year-old male isn't normal for a 60-year-old female.
